School personnel can administer prescribed medications to students under the following conditions:

1. The medication is required to maintain the child’s health.


2. The medication distribution is requested by the child’s parent in writing on the     provided form.

3. The medication is prescribed by a health care provider.

4. The medication is in tablet form. Liquid medications cannot be measured.

5. Medication must be in the original container with the child’s name.

Each prescription medication to be administered by school personnel must be accompanied by a completed form from the parent detailing the dosage to be administered and the time(s) for administration. Medication forms can be obtained from the office. Medication will be administered from 11:30 - 12:00. Over the counter medications to be administered by school personnel should be requested by the parent or legal guardian. A completed form from the parent detailing the dosage to be administered, the time(s) for administration and the ailment for which the medication is being dispensed must accompany the request. These forms, likewise, can be obtained from the office.

CLINIC

If a child becomes ill at school, his/her temperature will be taken by the teacher or office personnel. Children with a temperature of 100 or more will be sent home. Parents will be called any time a child has any kind of head injury. PLEASE PROVIDE US CORRECT PHONE NUMBERS, CHANGES IN YOUR PHONE NUMBERS AND SOMEONE WE CAN CALL IN AN EMERGENCY. Keep your child home if he/she has a contagious disease. Examples are:
